Drivers License Reinstatement Evaluation

If you are looking to have your drivers license reinstated after a suspension due to alcohol and/ or substance abuse charges, you can receive this evaluation here.

What you need to prepare

The evaluation will be scheduled for one hour. The cost for the evaluation is 200$, which must be paid in advance. Below is a list of documents to bring to the appointment.

OHAO form

Please bring your completed hearing request application, lab report, interlock report (if you have one), DA-4 form filled out by your doctor, documentation of previous substance abuse treatment, AA, or other group involvement. Please note ahead of time you need a referral for the 12 panel drug screen.
